FAB LAB JACKSON COUNTY SUMMER CAMP LIABILITY WAIVER & RELEASE FORM
Acknowledgment of Risk & Release of Liability

I, the undersigned participant (or parent/guardian of the minor participant), acknowledge that participation in Fab Lab Jackson County's Summer Camp, operated under the Jackson County School District, involves hands-on STEAM activities, including but not limited to 3D printing, laser engraving, robotics, and other interactive technologies. I understand that these activities carry inherent risks, including but not limited to potential injury, burns, allergic reactions, or property damage.

I agree that participation in the camp is voluntary and assume all risks associated with these activities. I acknowledge that Fab Lab Jackson County, the Jackson County School District, their employees, volunteers, affiliates, and sponsors are not responsible for any injuries,
accidents, or damages that may occur before, during, or after camp participation.

Waiver of Liability
I hereby release, discharge, and hold harmless Fab Lab Jackson County, the Jackson County School District, their employees, agents, affiliates, and sponsors from all claims, liabilities, damages, or causes of action, including but not limited to personal injury, illness, property damage, or financial loss arising from participation in camp activities.

Medical Authorization
In the event of a medical emergency, I authorize Fab Lab Jackson County and Jackson County School District staff to seek necessary medical treatment for my child/myself. I understand that I am responsible for any medical expenses incurred as a result of participation in the program.
